Hi,

Could anybody please direct me to some templates or examples of COSH and RA forms. My current models are less than satisfactory.

Hi chalky,

Try the hse website, there is a raft of information on there, spend an hour or so looking around and then go back to it periodically to get specific info.
They have example risk assessments but the nearest that I could see was this one. The principle is the same though.
